Overview

About Unshaven Hair Transplant

In an unshaven transplant, the team works around your existing hair instead of trimming the whole head. The donor area may be trimmed only in narrow strips that longer hair can cover, and grafts are placed between existing hairs in the recipient area.

This approach suits people who want to return to work or daily life with fewer visible signs of a procedure. It is more time-consuming and technically demanding, so it is usually limited to smaller graft numbers.

Suitability

Who it may be suitable for

  • Early to moderate thinning, often at the hairline or temples
  • Smaller planned graft numbers
  • Enough existing hair length to cover trimmed donor strips
  • A strong preference for discretion during recovery

These are general points only. Whether a treatment is suitable for you is decided by the treating clinician after reviewing your photos, medical history and, where needed, an in-person examination.

The process

How it works

  1. Review with longer hair

You share photos with your hair at its usual length and with it parted, so the surgeon can judge coverage and the donor area.

  2. Discreet donor preparation

Where needed, narrow strips of the donor area are trimmed in a pattern that the surrounding hair will cover.

  3. Extraction

Follicles are removed individually under local anaesthesia.

  4. Placement between existing hair

Grafts are implanted carefully among your current hairs, usually with an implanter pen, to avoid damaging them.

  5. Aftercare

The team reviews the area and explains how to wash and style your hair while the grafts settle.

Recovery

What recovery usually looks like

  1. Days 1–3

Some tightness, mild swelling around the forehead or eyes and small scabs around the grafts are common. You sleep with your head slightly raised and follow the clinic’s instructions for the first gentle wash, usually the day after the procedure.

  2. Days 7–10

The small crusts soften and come away with careful washing. Redness fades gradually. Many people return to desk-based work in this window, though more physical jobs may need longer.

  3. Weeks 2–8

Most transplanted hairs shed. This is an expected phase: the follicles stay in place beneath the skin and enter a resting period before regrowing.

  4. Months 3–6

New growth usually starts to appear, often fine and uneven at first, then gradually thickening.

  5. Months 12–18

The result is usually assessed at around a year. Density at the crown can take longer to mature than at the hairline.

Timelines vary between people. Always follow the aftercare instructions from your treating clinic.

Good to know

Risks and considerations

  • Because sessions are slower and more delicate, unshaven plans usually allow fewer grafts than a fully trimmed session. A large area may need a different approach.
  • The donor area is a limited resource. A responsible plan spreads it across your likely needs over the years, not only the area that bothers you most today.
  • A transplant does not stop ongoing hair loss in the surrounding native hair. The surgeon may discuss medical treatment to help protect what you have.
  • Common, usually temporary effects include swelling, tenderness, numbness in the donor or recipient area and small pimple-like spots (folliculitis). Infection and noticeable scarring are uncommon but possible.
  • Growth can be uneven, and some people choose a second, smaller session to refine density. Results vary from person to person and cannot be guaranteed.
  • Smoking, some medicines and certain medical conditions can affect healing. Tell the clinic about your full medical history and current medication before any plan is confirmed.
FAQ

Common questions

Will people be able to tell I had a procedure?

Longer hair hides much of the work, but some redness or small crusts may be visible up close in the first week or so. How discreet it is depends on your hair length, colour and the area treated.

Can any transplant be done unshaven?

No. It works best for smaller areas and graft numbers. The surgeon advises whether it is realistic for your plan.

How long should I plan to stay in Istanbul?

Most graft-based procedures are planned around three nights: arrival and pre-procedure checks, the procedure day, and a review with the first wash before you fly. Your coordinator confirms the exact schedule once the clinic has set the date.

How is the cost decided?

The price depends on the plan the surgeon recommends after reviewing your case, mainly the area to be treated and the technique. You receive a written, itemised quote before anything is booked, with no obligation to proceed.
